Inner vs outer thigh lift, how your surgeon decides
The choice starts with where the loose skin sits, not with what you ask for. Your surgeon pinches the skin on the inner thigh, checks how far it extends toward the knee, and looks separately at the outer thigh and hip. Loose skin limited to the inner thigh usually points to an inner (medial) thigh lift; skin that is loose both across and down the leg, often after large weight loss, is what the vertical technique is designed for. Outer thigh and hip laxity is treated separately, sometimes alongside a BBL.
Liposuction is usually added rather than chosen instead — it reduces the fat layer while the lift removes the skin envelope around it, which is why most JFS thigh lift patients leave with both done in the same operation. How much skin comes off, and how long the incision needs to run, is measured by pinching the loose tissue with you standing, then marked immediately before surgery so the final scar sits as low and short as your anatomy allows.

How a thigh lift is done, step by step
From the moment you go to sleep to the moment your dressing goes on, here is what actually happens during an inner thigh lift combined with liposuction.
Your thigh lift starts under general anaesthesia, so you are fully asleep for the whole operation. Before you go under, your surgeon marks your thighs while you are still standing — tracing the incision line in the groin crease, how far it needs to extend toward the knee, and where liposuction will contour the surrounding fat. Those marks carry forward exactly what you and your surgeon agreed at the Day 2 consultation.
The operation itself begins with liposuction of the inner thigh, removing fat through small, hidden openings before any skin is cut. Your surgeon then makes the incision along the marked groin crease, lifting the loose skin away from the tissue beneath it to see exactly how much needs to come off.

Inner thigh lift incision and lift
The excess skin is trimmed to the marked line, and the remaining tissue is anchored with deep sutures to the strong ligament near the groin, so the lift holds without the skin closure carrying the tension.
The incision is closed in layers — deeper sutures take the tension off the skin so the scar heals as thin as possible, then finer stitches close the surface along the groin crease. A thin drain is often placed on each side to clear fluid for the first few days, particularly when liposuction has been combined with the lift. Before you wake up, your compression garment is already in place, supporting the area from the moment you open your eyes.
