A gastric sleeve, or sleeve gastrectomy, removes the larger outer curve of the stomach through 5 or 6 small keyhole cuts, leaving a narrow tube roughly the shape of a banana. In Izmir it takes around 1 to 2 hours under general anaesthesia, followed by 2 to 3 nights in hospital.

Because the new stomach holds far less, you feel full after a small plate. The part removed is also where most ghrelin, the hormone that drives hunger, is made, so many people notice their appetite drops in the first months. Food still travels the usual route, through the stomach outlet into the small intestine, so the way your body absorbs nutrients changes less than after a bypass, although daily vitamin supplements are still recommended for life.

The sleeve is not reversible, since the removed part of the stomach is taken out of the body. If reflux becomes a problem in later years, a sleeve can usually be converted into a gastric bypass. That is why your surgeon asks about heartburn before recommending a sleeve.

Gastric bypass

A gastric bypass divides the stomach into a small pouch, about the size of an egg, and connects it straight to the small intestine, so food skips the rest of the stomach and the first section of the gut. Expect around 2 to 3 hours of keyhole surgery and 2 to 3 nights in hospital in Izmir.

In a Roux-en-Y gastric bypass, pronounced "roo-en-why", the small intestine is divided and one end is brought up to the pouch; the other end is joined further down, forming the Y shape that gives the operation its name. In a mini gastric bypass, also called a one-anastomosis bypass, the pouch is longer and narrower and is connected to a loop of intestine with a single join, which makes the operation a little shorter.

Mini bypass vs Roux-en-Y is decided at your consultation, based on your reflux and BMI. Both versions need lifelong vitamin B12, iron, calcium and vitamin D, plus blood tests at least yearly. A revisional gastric bypass, used to correct or convert an earlier sleeve or band, is a separate and longer operation; whether it is offered for your case is confirmed by your surgeon.

A gastric balloon is placed through the mouth with an endoscope, a thin flexible camera, in a procedure of around 20 to 30 minutes under sedation. There are no incisions, nothing is removed, and the same Day 2 tests in Izmir come first.

Filled with sterile saline, the balloon takes up space and slows how quickly the stomach empties, so you feel full sooner. The first two or three days often bring nausea and cramping, eased with medication. Your surgeon tells you when you can return to the hotel after observation.

Depending on the type used, a balloon stays in place for six to twelve months and is then removed with a second short endoscopy. That temporary nature is the point: the balloon gives you a period of reduced appetite in which to change how you eat, supported by your dietitian. The operation

How obesity surgery is done, step by step

On Day 3 of your 8 days in Izmir, a sleeve or bypass takes around 1 to 3 hours through 5 or 6 keyhole cuts; a balloon takes 20 to 30 minutes through the mouth.

A gastric sleeve or bypass is done under general anaesthesia, so you are fully asleep throughout. Compression stockings or calf pumps go on first, to protect against blood clots. Your surgeon makes 5 or 6 small cuts, each roughly one to two centimetres long, and gently inflates the abdomen with carbon dioxide to create space to work with a camera and long, slim instruments.

For a gastric sleeve, a thin tube called a bougie is passed down into the stomach to act as a guide. Your surgeon then fires a stapler along its edge, from near the stomach outlet up towards the top, dividing off the outer curve; the staple line is often reinforced, and the removed part is taken out through one of the keyhole cuts. For a gastric bypass, a small pouch is stapled off at the top of the stomach, the small intestine is divided, and one end is brought up and joined to the pouch; in a Roux-en-Y bypass a second join lower down completes the Y.

Before closing, the surgeon checks every staple line and join for leaks, commonly by filling the new stomach with air or a coloured liquid and watching for bubbles or seepage. An anaesthetist monitors your heart rate, oxygen and blood pressure throughout.

A gastric balloon follows a different path. Under sedation, the endoscope checks the stomach lining, then the deflated balloon is guided down and filled with sterile saline through a thin tube. The whole procedure takes around 20 to 30 minutes and leaves no scars.

After a sleeve or bypass, you wake up in a recovery area where nurses watch your breathing, pain relief and blood pressure before you move to your hospital room. You are encouraged to sit up and take a short walk the same evening, and your first sips of water come once your surgeon is satisfied the staple lines are holding, usually the next day.

Recovery after a sleeve or bypass is shaped less by the small keyhole wounds than by your new stomach. The first month is about protecting the staple lines while they heal, which is why diet after a gastric sleeve moves through four stages: clear liquids, then full liquids and protein drinks, then pureed food, then soft food. Normal textures usually return from around six to eight weeks, in much smaller portions than before.

Two habits matter from day one. Sip at least one and a half to two litres of fluid across the day, never gulping, and keep drinks separate from meals by around 30 minutes so food has room. Protein comes first on every plate, to help healing and protect muscle.

What's the difference between a gastric sleeve and a gastric bypass?

A gastric sleeve removes the outer curve of the stomach and leaves a narrow tube, without changing the route food takes. A gastric bypass creates a small stomach pouch and joins it directly to the small intestine, so food skips most of the stomach and the first part of the intestine. Bypass is often discussed when you have severe reflux or type 2 diabetes.

How long does a gastric balloon stay in?

Most gastric balloons stay in place for six to twelve months, depending on the type used, and are then removed with a second short endoscopy. Your surgeon confirms which balloon is used and how long it stays in at your consultation.

When can I fly home after obesity surgery?

After your surgeon's final check-up, usually around day 7 or 8 of your trip for a sleeve or bypass. Wear compression stockings, walk in the aisle every hour, keep sipping water and avoid lifting heavy luggage. After a gastric balloon, your surgeon tells you when it is safe to fly home.

How soon will I see results after obesity surgery?

Most people notice their weight changing in the first weeks, while their diet is still mostly liquid. Weight loss usually continues over the following year to eighteen months after a sleeve or bypass, and is slower and smaller with a balloon. Results vary from patient to patient. Your surgeon will tell you what is realistic for your body at the consultation.
