BBL with liposuction in Istanbul and Izmir, Turkey, shaped with your own fat
BBL with liposuction removes fat from areas such as the waist, back and flanks, then uses it to add natural shape to your buttocks — something diet or exercise alone cannot reshape once your body's fat pattern is set. In Istanbul or Izmir, Turkey, your JFS package includes one night in hospital and four in the hotel, from £3,050.

Am I a candidate?
Who is a good candidate for BBL with liposuction?
Diet and exercise can reduce fat all over the body, but they cannot move it from one area to another. BBL with liposuction is the only procedure that both contours stubborn fat and uses it to add shape elsewhere, in a single operation lasting roughly two to three hours.
Buttock volume loss, why exercise alone cannot add shape
Squats and glute exercises can strengthen the muscle underneath, but they do not add the soft, rounded volume that sits over it — that layer is fat, and genetics decide how much of it collects there naturally. At the same time, some areas such as the waist, back and flanks hold onto fat even at a stable, healthy weight, resisting diet and cardio in a way that frustrates many patients before they ever consider surgery. BBL with liposuction treats both problems in one operation: fat is removed from the areas that will not slim down on their own, then reinjected into the buttocks to build shape that exercise cannot create from nothing.
Signs BBL with liposuction may suit you
- Stubborn fat on the waist, back or flanks that has not responded to diet or exercise.
- You want more volume or curve in your buttocks than exercise alone has been able to build.
- You carry enough donor fat in the areas being treated for your surgeon to harvest a useful amount.
- Your weight has been stable for several months and you are not planning a major weight change, since both affect the transferred fat.
- You don't smoke, or you're able to give it up well ahead of surgery and during recovery — nicotine slows healing and raises the chance of complications.
| Fat transfer (BBL) | Buttock implants | |
|---|---|---|
| What's added | Your own fat, harvested by liposuction | A solid silicone implant |
| Needs donor fat | Yes — some areas are also contoured as a result | No donor fat required |
| How it feels | Soft, moves naturally like your own tissue | Firmer, placed under or in the muscle |
| Incision | Small cannula entry points only | A single incision to place the implant |
| Typical candidate | Enough donor fat, wants a natural look and feel | Very little donor fat available |
Most JFS Clinic patients choose fat transfer; buttock implants are a different operation and are discussed separately at your consultation.
What to discuss at your BBL with liposuction consultation
- Which areas have enough donor fat, and how much your surgeon expects to harvest.
- The shape and volume you're hoping for, and what your donor fat realistically allows.
- Whether standard or VASER-assisted liposuction fits your fat and your goals (see the comparison below).
- Your full medical history, including smoking, blood thinners and any previous body surgery.
- How you'll need to lie and sit for the first few weeks, so you can plan your travel home around it.
This is general guidance, not a diagnosis. The final call always sits with your surgeon, once they've reviewed your photos and medical background.
Not every BBL is the same
Types of BBL with liposuction, standard and VASER-assisted technique
"BBL" covers how the fat is harvested and purified as much as how it is placed. Your surgeon picks the technique based on how fibrous your fat is and how much volume you want transferred — not the other way around, across the two to three hours a combined case usually takes.
- 1Small entry points made
- 2Fat harvested from the flanks
- 3Fat moved to hips and buttocks
- 4Waist-to-hip curve refined
How BBL with liposuction is done
| Technique | Best for | Incision | Main difference |
|---|---|---|---|
| Standard liposuction + fat transfer | Most patients with enough soft donor fat in the waist, back or flanks | Small cannula entry points, usually along the waistline | Fat removed by hand with a cannula, processed, then injected |
| Waist and back liposuction with fat transferPackage technique | Patients wanting a smaller waist and more volume behind in one operation | Cannula entry points at the waist and back | Same fat used to contour the waist is reinjected into the buttocks |
| VASER-assisted liposuction + fat transfer | Fibrous fat that is harder to remove by hand alone | Same small entry points, with ultrasound energy used first | Ultrasound loosens the fat before it is removed, which can protect fragile fat cells for transfer |
| Skinny BBL | Slim patients with limited donor fat who still want moderate shape change | Fewer, smaller entry points, since less fat is harvested | Smaller transferred volume, often combined with a thinner-fat processing method |
| BBL with extended liposuction | Patients who also want the thighs or abdomen contoured in the same operation | Additional entry points at the extra treated areas | More donor fat harvested overall, usually a longer operation |
How much fat can be harvested and transferred always depends on your own donor fat — your surgeon confirms realistic volume at your consultation.
Standard vs VASER-assisted BBL, how your surgeon decides
The choice is made at the consultation, by pinching and assessing the fat in your waist, back and flanks. Soft, easily compressible fat usually works well with standard liposuction, where a thin cannula moved by hand loosens and removes it. Firmer, more fibrous fat — more common in some areas and in some patients — can be harder to remove cleanly by hand, which is where VASER-assisted liposuction is considered: ultrasound energy loosens the fat first, so the cannula removes it with less force and, some surgeons believe, less damage to the fat cells that will later be transferred.
How much fat is available also shapes the plan. Your surgeon estimates your donor fat by assessing the waist, back and flanks together, since combining areas usually yields more volume for transfer than treating one area alone. Patients with limited donor fat are offered a smaller, proportional result — sometimes called a skinny BBL — rather than a volume that their own tissue cannot realistically support.
Operating time is also measured here: harvesting enough fat for a meaningful transfer, purifying it, and then carefully injecting it in small amounts across many passes takes longer than liposuction alone. A combined waist-and-back case with a standard transfer typically takes around two to three hours; adding VASER preparation or extra donor areas extends that time further.

Fly to Istanbul or Izmir
Your 6 days in Turkey begin. Nothing is finalised until blood tests and an in-person consultation have both taken place.
What can delay your BBL with liposuction date
Being able to travel within a week does not mean you should if your donor fat or your health is not ready yet. Several factors shape your safest surgery date, and your surgeon reviews each one before confirming it. Should your blood tests or the Day 2 consultation suggest that holding off is wiser, the date is adjusted. Your safety comes before the booking.
| Factor | Why it matters | What usually happens |
|---|---|---|
| Smoking or nicotine | Nicotine narrows blood vessels and slows healing in both the donor areas and the grafted fat | You are asked to stop for several weeks before and after surgery |
| Very low donor fat | Not enough fat in the waist, back or flanks limits how much volume can be transferred | Your surgeon plans a smaller, proportional result or suggests waiting |
| Weight still changing | Transferred fat changes size with the rest of your body, so losing weight later reduces it | Surgery is planned once your weight has been stable for a while |
| Blood thinners and some supplements | They can increase bleeding during and after surgery | Your surgeon tells you what to pause and when. Never stop prescribed medication without your own doctor's advice |
| Plans for pregnancy | Pregnancy changes body shape and fat distribution | Many people wait until their family is complete |
If you live in the UK, the NHS does not fund a BBL with liposuction, because it is a cosmetic operation, so most people compare private clinics at home and abroad. Wherever you go, ask three things before you book: how the surgeon avoids injecting fat into the muscle, what happens if a test result delays your surgery, and who looks after you once you are home.

What changes the price: adding more donor or treatment areas (such as the thighs or abdomen), choosing VASER-assisted liposuction, or combining your BBL with a tummy tuck in the same operation. A smaller, skinny BBL with limited donor fat can sometimes cost less than the standard package; adding areas or combining procedures typically costs more because of the extra surgical time and, sometimes, a second hospital night. The exact figure is set once your surgeon has gone through your photos and the notes from your Day 2 consultation.
The operation
How a BBL with liposuction is done, step by step
From the moment you go to sleep to the moment your dressing goes on, here is what actually happens during a BBL with liposuction, over roughly two to three hours.
Your BBL with liposuction starts under general anaesthesia, so you are fully asleep for the whole operation. Before you go under, your surgeon marks your waist, back and flanks while you are still standing — tracing exactly where fat will be harvested, and marking the buttocks to plan where volume is needed most. Those markings simply carry forward the plan you and your surgeon agreed on two days earlier.
The operation begins with liposuction. Small cannula entry points are made at the marked areas, and a thin cannula moves back and forth to loosen and remove fat — by hand for standard liposuction, or with ultrasound energy first if the VASER-assisted technique is being used for fibrous areas. Your surgeon removes fat evenly across the waist and back, checking the contour as they go, since harvesting too much from one spot can leave an uneven surface.
Inside the transfer
BBL fat transfer in four steps
- 1Fat marked and harvestedLiposuction removes fat from the waist and back.
- 2Fat purified and preparedHarvested fat is filtered before it is used.
- 3Fat injected beneath the skinSmall amounts placed in layers, never into the muscle.
- 4Shape checked and refinedVolume compared from every angle before closing.
BBL fat transfer, step by step
The harvested fat is then processed to separate usable fat cells from blood and fluid — usually by settling or gentle centrifuging — before it is loaded into syringes for transfer. Only the cleanest fat is used, since damaged or non-viable fat cells do not survive being reinjected and can affect the final result.
Placement is the step surgeons are most careful about. Op. Dr. Onur Evren Yılmaz injects the fat in small amounts, in layers, through a blunt cannula moved along many separate passes rather than one large injection — and critically, into the subcutaneous layer under the skin, never into the gluteal muscle itself. This technique exists specifically to avoid the rare but serious complication linked with buttock fat transfer, which happens when fat is injected deep enough to enter a vein inside the muscle. Working only above the muscle, in thin, well-spread layers, is the recognised way surgeons reduce that risk while still building a rounded, natural shape.
Throughout the operation an anaesthetist watches your heart rate, oxygen levels and blood pressure, adjusting your anaesthesia as the surgery moves between stages — harvesting, processing and injecting the fat each take a different amount of time depending on how much volume is being transferred. A combined waist-and-back case with a standard fat transfer typically takes around two to three hours in total; adding VASER preparation, extra donor areas, or a combined tummy tuck extends that time further.
Once the dressings and your compression garment are on, you are moved to a recovery area where nursing staff watch you wake from the anaesthetic before you go to your hospital room for your one night under observation. You are positioned lying on your front or side from the very start, since avoiding pressure on the buttocks protects the freshly transferred fat while it establishes its own blood supply in the first critical days.
Recovery, day by day
BBL with liposuction recovery time, week by week
Move the slider to any point across the first 6 to 12 months after surgery. It shows roughly how people tend to feel at that point, what's usually fine to do and what to steer clear of.
Recovery from BBL with liposuction happens in three broad phases. The first two to three weeks are about protecting the transferred fat — you avoid sitting directly on your buttocks, use a BBL pillow when you must sit, and let the compression garment support the liposuction areas while they settle. Weeks three to six are about swelling going down in both the donor areas and the buttocks, which is usually when sitting starts to feel normal again. From six weeks onward, the focus shifts from healing to the final shape settling in.
Not sitting directly matters because the newly transferred fat needs to develop its own blood supply before it can survive long term; pressure in the early weeks can compress that fat and reduce how much of it takes. Your coordinator shows you how to use the BBL pillow — a cushion shaped to keep weight off the buttocks and onto the thighs and lower back instead — for the car, the hotel bed and eventually the flight home.
Typical timeline for a combined waist-and-back BBL. Your surgeon may give you a different plan, especially if a tummy tuck is combined.
| Activity | Week 1 | Week 2 | Weeks 3–4 | Week 6 | Months 3–6 |
|---|---|---|---|---|---|
| Sitting | Avoid completely, use the BBL pillow for any travel | Short periods with the pillow | Longer periods, pillow for firm seats or long journeys | Normal sitting, no restriction | No restriction |
| Walking | Short, slow walks with help | Walking further each day on your own | Longer walks, no restriction | Normal walking pace, no restriction | No restriction |
| Desk work | Not yet — the focus is on rest and lying flat | Possible, using the BBL pillow | Full days comfortable, less reliance on the pillow | No restriction | No restriction |
| Driving | Avoid — reaction times are affected by pain relief | Confirmed by your surgeon at your check-up | Set by your surgeon after your check-up | Cleared by your surgeon as you heal | No restriction |
| Gym | Not yet | Avoid — no swimming or squats yet | Still avoid — no running or heavy lifting | Gradual return, starting light | Normal exercise, as cleared by your surgeon |
| Garment | Worn at all times | Worn at all times | Worn at all times | Coming off, or worn only part-time | As advised by your surgeon |
Typical timeline for a combined waist-and-back BBL, built from the recovery guidance above. Where a cell says "Confirmed by your surgeon at your check-up," individual healing varies too much to generalise — bring it up at your consultation.
A returning patient's experience
Tülin has returned to JFS Clinic several times, including for a BBL. In this video, filmed at a later follow-up check-up, she describes how recovery and check-ups go with the same surgeon.
I had my surgery a week ago now and I am very happy. Everything is going well. I came for my check-up with Dr. Onur Evren Yılmaz and the results are wonderful.
What to expect
BBL with liposuction results and scar healing
Most of the early swelling is gone within about six weeks, though the buttocks keep settling for several months as the transferred fat establishes its own blood supply and the remaining swelling resolves. Many patients notice the new waistline early, while the very final shape behind — how rounded, how high, how it sits — takes closer to three to six months to show fully.
Not every fat cell transferred survives the move; the fat that does take hold behaves like the rest of your body fat from then on, which is why keeping your weight stable afterwards is the main way patients protect their result. Liposuction leaves a few small marks at the cannula entry points, which start pink and gradually fade over the following months to faint lines most patients find easy to cover.
Results vary from patient to patient. Your surgeon will tell you what is realistic for your body at the consultation.

Questions
BBL with liposuction in Turkey, frequently asked questions
How much does a BBL with liposuction cost in Turkey?
At JFS Clinic, a BBL with waist and back liposuction is listed from £3,050 (€3,500 / $4,050), covering surgery, one night in hospital, four nights in the hotel and all transfers. Adding further areas or combining it with a tummy tuck changes the price; your surgeon confirms the final figure after reviewing your photos.
See all package prices →How soon can I have BBL with liposuction in Turkey?
Most JFS patients travel for their BBL with liposuction within roughly seven days of first getting in touch. You send photos of your waist, back and buttocks, receive a personal plan with the dates available, then choose when to travel. Your surgeon only finalises the date once your blood tests and the in-person consultation in Istanbul or Izmir are done, and pushes it back if that would be the safer choice.
From first message to surgery →Can you get a BBL in Turkey?
Yes, when it is carried out by an experienced surgeon in a licensed hospital. Op. Dr. Onur Evren Yılmaz places the transferred fat under the skin rather than into the gluteal muscle, which is the technique surgeons use to avoid the rare but serious complication linked with buttock fat transfer. A Day 2 blood panel and pre-operative assessment check that you're fit for a general anaesthetic before any surgery date is locked in. Ask about your surgeon's technique, the hospital's licence and the aftercare plan before you book.
What is the best country for a BBL?
No country can honestly claim to be the best for every patient, so compare clinics on standard points: the surgeon's specialist training and experience with fat grafting and liposuction, a hospital licensed for international patients, a written price stating what it includes, and follow-up once you are home. At JFS Clinic you see your surgeon's name before booking and meet them before surgery. Op. Dr. Onur Evren Yılmaz, a plastic surgeon who completed his specialist training in 2012, operates, and your coordinator passes questions and photos from home to him on WhatsApp.
How does a BBL look after 10 years?
Transferred fat that survives the first few months behaves like the rest of your body fat, so it can shrink or grow slightly if your weight changes over the years. Keeping your weight stable is the main way patients keep their shape close to the original result. Results vary from patient to patient; your surgeon will tell you what is realistic for your body at the consultation.
On a scale of 1 to 10, how painful is a BBL with liposuction?
You are asleep for the operation itself. Afterwards, most patients describe the liposuction areas as sore and tight, similar to intense muscle soreness, while the buttocks feel firm and swollen rather than sharply painful. Not being able to sit normally for the first few weeks is usually described as more inconvenient than painful. Pain relief is provided, and how it feels varies from person to person.
What's the difference between standard and VASER-assisted BBL?
Standard liposuction removes fat with a cannula moved by hand, which suits most patients with soft, easily accessed fat. VASER-assisted liposuction uses ultrasound energy to loosen fibrous fat first, which can help in areas that are harder to treat by hand. Your surgeon decides which suits your waist and back at the consultation.
Can a BBL be combined with a tummy tuck?
Sometimes, yes. Combining a BBL with a tummy tuck in the same operation is possible for some patients, though it adds surgical time and sometimes a second hospital night. Your surgeon decides based on your health, your donor fat and how long the combined surgery would safely take.
How long are the scars and do they fade?
Liposuction leaves a few small marks, roughly a centimetre each, at the cannula entry points around the waist, back and flanks, and fat is injected into the buttocks through similarly small points. They start pink, then gradually fade over the following months to faint marks that are easy to miss under swimwear.
How soon will I see my final result?
Initial swelling takes several weeks to settle, and most people see their new shape taking form by around three months, once the transferred fat has settled into place. The area continues to refine for up to a year. Results vary from patient to patient. Your surgeon will tell you what is realistic for your body at the consultation.
