Penile filler vs fat transfer vs implant, how your surgeon decides
The examination starts with what you are actually seeing. Your surgeon looks at you standing and lying down, checks whether a fat pad or skin webbing is hiding part of the shaft, and feels the skin over the shaft to judge how much room there is under it. If the shaft is partly hidden, pubic liposuction or a webbing release usually comes first, because adding volume to a penis that is still partly covered changes little.
If girth is the concern, hyaluronic acid filler is the most common first step. It is placed under the skin through one or two tiny entry points, can be adjusted at a follow-up visit, and is slowly absorbed by the body, which also means the change is reversible. Fat transfer uses your own tissue instead, taken by liposuction from the abdomen or flanks, cleaned and injected in thin layers. A share of it does not survive the move, so a second session is sometimes planned. A silicone sleeve implant is a bigger operation under general anaesthesia and is considered only after the risks, including infection and removal, have been discussed openly.
Ligament release is the technique men ask about most and the one surgeons are most cautious with. Cutting the suspensory ligament lets more of the shaft hang forward at rest, but it does not lengthen the erect penis, and the erection may point lower afterwards. Most surgeons only offer it alongside a clear plan for scar care and a period of gentle traction afterwards.
Scrotoplasty and scrotal lift, a separate procedure
Scrotoplasty treats the scrotum rather than the penis. With age, heat or weight change, scrotal skin can stretch so it hangs low, rubs during sport or shows through clothing. The operation removes a strip of excess skin, usually along the natural midline seam, and closes it with dissolvable stitches, so the scar follows a line that is already there. The same approach can release penoscrotal webbing, where skin joins the underside of the shaft too high and makes it look shorter. Testicles and fertility are not affected by a skin-only lift.

How penile enlargement surgery is done, step by step
Below is what happens during penile girth enlargement with filler or fat, the most common request, followed by how ligament release and scrotoplasty differ.
On the morning of your procedure, your surgeon goes over the plan you agreed on Day 2 and marks the treatment area while you are standing, because the way the penis and scrotum hang changes once you lie down. Filler is usually done under local anaesthesia, after a numbing cream has worked for around 30 minutes. Fat transfer, ligament release and an implant are done under general anaesthesia or local anaesthesia with sedation, which the anaesthetist explains to you beforehand.
For girth enlargement, a tiny entry point is made at the base of the shaft, in a natural crease where the mark is hard to see once healed. A blunt cannula, rather than a sharp needle, is used to lay filler or purified fat in thin, even layers between the skin and the deeper tissue, working around the shaft rather than in one place. Penile enlargement with filler or fat
Ligament release follows a different route. A short incision of a few centimetres is made at the base of the penis, just under the pubic hair line. Your surgeon separates the suspensory ligament from the pubic bone so more of the shaft can hang forward, then places a small layer of fat or a spacer stitch so the ligament does not simply reattach in the same position. The skin is closed in layers with dissolvable stitches.
