Diet and exercise can shrink fat cells across the body, but they cannot target one stubborn pocket on its own — genetics decide where fat clings on, not how many sit-ups you do. Liposuction is the treatment that removes fat from a chosen area directly.

Localised fat deposits, why diet and exercise can't remove them

Fat cells in areas like the lower abdomen, flanks, inner thighs or under the chin can be more resistant to diet and exercise than fat elsewhere on the body, often because of genetics, hormones or how fat is distributed as you age. Once you reach a stable, healthy weight, these pockets can still sit there unchanged, independent of your overall body fat. Exercise can tone the muscle underneath, but it cannot shrink the fat layer sitting on top of it in one specific place — removing the fat cells directly is what liposuction does.

Tumescent vs Vaser liposuction, how your surgeon decides

The choice is not about which technique sounds more advanced, it is about what your surgeon finds at the consultation. Pinching the treated area shows how fibrous or soft the fat is, and fibrous areas — often the flanks, back or male chest — tend to respond well to the extra loosening that Vaser provides. Soft, straightforward pockets on the abdomen or thighs often come away just as well with standard tumescent technique alone.

Liposuction is often added to another operation rather than chosen on its own. On a tummy tuck, it contours the waist and flanks that the skin-removal part of the surgery does not reach. On a BBL, it is the source of the fat that gets transferred, so your surgeon also checks whether you have enough fat in the areas you want slimmed to make a worthwhile transfer.

How much is removed is measured in millilitres of fat, and your surgeon works to a safe upper limit for a single session based on your weight and health. How liposuction is done, step by step

From the moment you go to sleep to the moment your garment goes on, here is what actually happens during a single-area liposuction procedure.

Your liposuction usually takes place under general anaesthesia for larger or multiple areas, or local anaesthesia with light sedation for one smaller area. Before you go under, your surgeon marks the treatment area while you are standing — tracing the exact zones to be treated and the small points where the cannula will enter, based on the plan agreed on Day 2.

The procedure begins with a diluted anaesthetic and saline fluid injected into the fat layer through the same small entry points, a technique called tumescent infiltration. This fluid numbs the area, shrinks the small blood vessels running through the fat, and makes the fat easier to remove with less bleeding and bruising afterwards.

Liposuction fat removal

Working through the same tiny entry points, your surgeon passes a thin, blunt-tipped cannula back and forth through the fat layer, drawing it out with gentle suction. The movement is even and controlled, following the contour marked earlier, so the result is a smooth transition rather than a hollow or uneven patch. How much fat comes out depends on the size of the area and how much was agreed beforehand, always kept within a safe limit for your weight and health.

Once enough fat has been removed to reach the planned shape, the tiny entry points are closed with a stitch or two, or sometimes left to close on their own since they are so small. A compression garment goes on in theatre, straight over the treated area, to help control swelling and support the skin as it settles against its new contour. Drains are rarely needed for liposuction, unlike for a tummy tuck, because the entry points are small enough to let any remaining fluid drain on their own.

Throughout the procedure an anaesthetist watches your heart rate, oxygen levels and blood pressure, adjusting your anaesthesia if more than one area is being treated in the same session. A single area usually takes under an hour; treating several areas, or a full lipo 360, takes longer simply because there is more ground to cover, and your surgeon may plan a short pause partway through to check how the treated area is settling.

Once your garment and dressings are on, you are moved to a recovery area where nursing staff watch you wake from the anaesthetic before you go to your hospital room for your one night under observation. Most patients are up and walking slowly with help later the same evening or the next morning, which helps circulation and reduces swelling from the very start.

Recovery from liposuction happens faster than from surgery that removes skin, since there is no muscle repair and only small entry points to heal. The first week is about letting swelling and bruising settle while the garment holds the new shape in place. Weeks two to six are about the treated area looking progressively smoother as swelling drops, which is usually when people feel ready for the gym again. From six weeks onward, the focus shifts from healing to the area settling into its final, more defined shape.

The garment is not just comfort — it controls swelling and helps the skin settle evenly against the new contour, which is why it stays on almost all day for the first week or two and only moves to part-time wear once your surgeon is confident the swelling has started to even out.
