Open vs closed rhinoplasty, how your surgeon decides
The choice is about how much your surgeon needs to see to reshape your cartilage safely, not what you ask for. Your surgeon examines your skin thickness, the width and symmetry of your tip, and whether a septoplasty is being added. A straightforward bump or profile correction usually keeps a closed technique on the table; tip reshaping, asymmetry, or a combined septoplasty moves the plan toward an open approach, since lifting the skin gives a direct view underneath.

How a rhinoplasty is done, step by step
A bridge and tip rhinoplasty takes around two hours under general anaesthesia; here is what happens from the first incision to the moment your splint goes on.
Your rhinoplasty starts under general anaesthesia, so you are fully asleep for the whole operation. Before you go under, your surgeon reviews your marked photos and measurements one last time, confirming the bridge line, the tip projection and whether a septoplasty for breathing is being added. They follow directly from the plan you and your surgeon agreed on Day 2.
The operation itself begins with the incision. In a closed rhinoplasty this sits entirely inside the nostrils; in an open rhinoplasty a small cut is also made across the columella, the strip of skin between the nostrils, so the skin covering the nose can be carefully lifted away from the bone and cartilage beneath it.

Rhinoplasty bridge and tip reshaping
Underneath the skin, your surgeon works on the bone and cartilage that actually gives the nose its shape. A visible bump is reduced by filing or trimming the bone and cartilage beneath it; narrowing the bridge, when needed, involves controlled fractures so the bones can be repositioned closer together. The tip is reshaped separately with sutures that narrow, rotate or support the cartilage rather than removing large amounts of it — part of why tip swelling takes longer to settle than the bridge.
If the septum — the wall down the middle of the nose — is deviated, it is straightened at the same time, by trimming or repositioning the crooked section while keeping enough structure to support the bridge. This is the step that improves airflow, carried out only when your surgeon's breathing assessment points to it.
Throughout, an anaesthetist watches your heart rate, oxygen levels and blood pressure, adjusting your anaesthesia as surgery moves between stages. A straightforward bridge and tip rhinoplasty typically takes around two hours in total; adding a septoplasty, cartilage grafting, or revising an earlier operation takes longer.
Once the cartilage and bone are in their new position, the skin is redraped back over the structure and the incisions are closed with fine sutures. A rigid splint is taped over the bridge to protect the new shape while the bone sets, and soft internal packing or splints may be placed inside the nostrils for the first day or two. You are then moved to a recovery area where nursing staff watch you wake from the anaesthetic before you go to your hospital room for the night.
