Vertical vs Wise-pattern reduction, how your surgeon decides
The choice is not about what you ask for, it is about what your breasts show at the consultation. Your surgeon pinches and measures the tissue, checks how far the nipple needs to travel to its new position, and looks at how your particular skin is likely to retract once the excess is gone. A smaller, moderate reduction with skin that still has good elasticity usually points to a vertical technique; a larger, heavier or lower-sitting breast, where the nipple needs to move a greater distance, points toward a Wise-pattern reduction with its extra crease incision.
Liposuction alone is sometimes enough when the main issue is fatty volume rather than loose skin or a low-sitting nipple — it removes fat through small ports without leaving a long scar, but it cannot lift or reshape the breast the way the other techniques do. For a very large reduction, the nipple's own blood supply may not stretch far enough to be repositioned safely while still attached, which is when a free nipple graft is considered instead, at the cost of nipple sensation and breastfeeding ability.
How much tissue needs to be removed is estimated in grams per breast, judged against your height, weight and chest measurements rather than a single fixed number. Your breast reduction starts under general anaesthesia, so you are fully asleep for the whole operation. Before you go under, your surgeon marks your chest while you are standing upright — tracing the new nipple position, the amount of skin to be removed and the exact incision pattern agreed at your consultation. These marks are a direct continuation of the plan from Day 2, drawn while gravity shows your surgeon exactly how your tissue sits.
The operation itself begins with the incision, cut along the lines marked a day earlier. For a Wise-pattern reduction this runs around the areola, down to the crease and along the crease itself; a vertical reduction skips the crease incision. Your surgeon removes a measured amount of glandular tissue, fat and skin, weighing what comes away from each side so the two breasts end up as close to symmetrical as your anatomy allows.

Breast reduction in four steps
Once enough tissue has been removed, the nipple and areola are repositioned higher on the breast — moved as a unit that stays attached to its own blood supply and nerve whenever the amount of reduction allows it, which is what keeps sensation and, for many patients, the ability to breastfeed afterwards. In a very large reduction, where the nipple would need to travel further than its blood supply can safely stretch, your surgeon may use a free nipple graft instead, removing and regrafting it as separate tissue; this trades some sensation and breastfeeding potential for a safer, more predictable result.
With the tissue removed and the nipple repositioned, the remaining skin is reshaped around the new, smaller breast mound and closed in layers — deeper sutures take the tension off the skin so the scar heals as thin as possible, then finer stitches close the surface. A thin drain is often placed under each breast to clear fluid for the first few days, and your support bra goes on before you wake up, holding the new shape steady from the moment you open your eyes.
Throughout the operation an anaesthetist watches your heart rate, oxygen levels and blood pressure, adjusting your anaesthesia as the surgery moves between stages — tissue removal, nipple repositioning and closure each take a different amount of time depending on how much work each side needs. A moderate reduction typically takes around two hours; a larger Wise-pattern reduction, or one needing a free nipple graft, takes longer simply because there is more surgical ground to cover on each side.
Once the dressing and support bra are on, you are moved to a recovery area where nursing staff watch you wake from the anaesthetic before you go to your hospital room for your night under observation. A drain, when one is used, sits under the dressing on each side to clear fluid from the wound bed; it is checked at each nursing round and usually comes out within the first week, once drainage has slowed enough for your surgeon to be confident the area will heal cleanly without it.
